If you are an existing patient, you can jump straight into scheduling here. If you are a new patient please call our office to schedule.
- Choose the service you want to be seen for.
- Select the provider and time slot you wish to do
- Follow the instructions to select your account
- Use email link to log into Patient Portal and complete patient paperwork.
You will receive an email after scheduling with an appointment confirmation and a link to access your patient portal

Becoming a patient is easy! Click the link to the right to give us basic information about yourself. You will receive an email with a link to your patient portal login. If you already created an account: select here to access your patient portal. Please note, to schedule your first appointment as a new patient, we ask that you contact our office.
- First, fill out the form.
- Then, you will receive an email link to your patient portal.
- Follow instructions to setup and login to your patient portal
Once you have logged into your portal, you are able to schedule appointments with our providers online, message our clinic members, view treatment plans, upload previous medical history (including lab work), and more.
