Direct Primary Care Memberships

Direct Primary Care Memberships (DPC)

Direct Primary Care is a membership plan for patients looking for quality, affordable primary care.  Instead of being charged per visit, the member pays a monthly fee and can be seen as needed for well or sick visits.  Benefits of DPC include:

  • Convenience.  You have a set monthly charge for your membership and do not have surprise bills for routine care.
  • Priority scheduling for sick visits.  Even if our schedule is full, we reserve time on our schedule each day for members with urgent needs.
  • Savings.
    • Lab fees for members are waived (this does not apply to specialty labs).
    • No charge for answering emails, sending refills, and other selected administrative tasks.
    • Having access to a primary care provider who knows you.
    • Reduces the need for costly visits to urgent care or the emergency room.
  • Please note that Direct Primary Care applies only to services provided at our office.  It does not replace the need for other insurance, but is a great option for those without traditional insurance or with high deductible plans.

    • Other Membership Plans

    DPC is a great option for your general health needs, but is not the best fit for everyone.  If you require more specialized or complex care, we have other plans that will better suit your needs. A representative of our clinic can help determine your level of care when you are signing up.

  • DPC Plus is for members with multiple health challenges that require more frequent follow-up.
  • DCC (Direct Complex Care) plans are for members being treated for illnesses such as borreliosis (Lyme disease) and related infections.
  • DCC Plus plans are for DCC members who need a higher level of follow-up or coordination with other providers.  This includes patients receiving IV therapy.

    Direct Complex Care (DCC) Membership Levels

    Cost

    DCC Individual (Adult)

    • Monthly visit with your primary provider
    • Monthly check-in with nurse
    • No additional charge if one additional visit is needed

    $250

    DCC Plus Individual (Adult)

    • All services in regular DCC plan
    • No additional charge for monthly safety labs (CBC and CMP) drawn at our office
    • No additional fees for administrative tasks up to one hour per month (prior authorizations, scheduling, ordering supplies, service coordination)
    • Monthly visit with naturopath or counselor

    $400

    DCC Individual (Child)

    • Monthly visit with your nurse practitioner and Angela (naturopath)
    • Monthly phone check-in
    • No additional charge for one additional visit if needed

    $300

    DCC Plus Individual (Child)

    • No additional charge for monthly safety labs (CBC and CMP) drawn at our office
    • No additional fees for administrative tasks up to one hour per month (prior authorizations, scheduling, ordering supplies, service coordination)
    • Monthly visit with counselor.  Counseling visit can be for the child or the parent

    DCC Families

    Ask us about the cost of adding additional family members to your DCC Plan

    		 

    When signing up for any of our membership plans, you will be asked to make a 6-month commitment.  After six months you may cancel at any time by giving written notice.
